Hi Greg,
it could either be the firmware or a faulty box. It seems that a few people have had problems with this modem at the firmware version that you have.
It could also mean that the modems shipped out with that initial firmware were faulty in some way and may need to be returned.
Either way you will have to contact your ISP and I bet it is BS and tell them the problems you are having. They maight email you a firmware upgrade or give you a new modem.
Remember to tell them that the Default Gateway is coming up as 0.0.0.0
