Usually registry cleaners are just trying to scare you into downloading, then buying them. Don't touch.
If you actually have a registry problem that really causes the computer to not work properly then you may need to work on the registry. If you haven't fiddled with it then this is unlikely. I wouldn't even run registry checking software if the PC is working OK. Registry cleaning is unlikely to make any practical improvement to a working PC, and if it goes wrong can be totally catastrophic.
If you are not absolutely sure what you are doing and have good backups don't touch the registry.
You certainly don't need to contemplate reinstalling anything based on what a warning says unless your PC is patently not working.
