Laptop won't boot from HD

A while back I developed a problem where my laptop won't do a cold boot from the hard drive - I must use the Toshiba Recovery disk, select the option to repair Windows Vista, then reboot and it works....until next time. The repair never takes. There is a boot file missing. When I look at the details, it is like the old DOS System.ini file, but only has 2 lines, one I believe simply states it is Windows Vista, the other says the path (like, C:\boot, or something like that). Anyone have any idea how I can fix this for good? I took pictures of the error messages, but don't have that .jpg with me right now (figures!). If this is too vague, will try to find the pic and post it again later.
