Vista Does Not Exit Full Screen

I'm having a strange issue where games and other apps will go full screen
without any problems but when I exit the app, Vista does not return to the
desktop. I'm left with 2 blank black screens and a pointer.
Running dual Samsung monitors on GeForce7600GT with all driver updates
applied.
Has anyone else run into this?
