Missing "X" in upper right corner of window

What might cause the "X" used to close a window to disappear?

Hi,
1. Which version of Windows? (Including SP level- SP1,SP2,etc.)

2. Which version of Internet Explorer?

3. Which programs are affected?

4. Did the problem appear immediately after one or more programs were
installed or updated?

5. What third-party security programs are installed and running? Are they
updated automatically?

6. From Start> Programs> Accessories> System tools> System restore, is a
recent restore point available?
